As a house rule I limit 'Greater Teleport' to 20,000 miles per level of caster. This would allow casters to travel from the planet they reside on to the 'moon' if the planet they reside has a moon any where from 220,000 miles to 260,000 miles (approximate distance from Earth to are moon).

In my personal campaign characters sometimes use this method to travel from one moon to another moon that both orbit a gas giant... as the moons orbits permit.

If one wants to limit distance but not as little as 20,000 miles per level, I would not exceed 1,000,000 miles per level of caster.

As for going from sphere to sphere... well while each crystal sphere sees itself as the prime material plane it should treat others crystal spheres as 'alternate primes' for purpose of greater teleport. Of course that opens another can of worms... Plane Shift.
